Tesamorelin is a synthetic peptide composed of 44 amino acids and represents a modified analog of growth hormone-releasing hormone (GHRH). A trans-3-hexenoic acid moiety has been added to its N-terminal region, enhancing its stability and prolonging its biological half-life compared with endogenous GHRH. Marketed as Egrifta® and Egrifta SV®, it remains the only FDA-approved GHRH analog specifically authorized for the treatment of excess visceral abdominal fat in adults affected by HIV-associated lipodystrophy.
Through selective stimulation of growth hormone secretion from the pituitary gland, Tesamorelin primarily reduces deep visceral adipose tissue while helping preserve both subcutaneous fat stores and lean body mass. Large randomized controlled trials have consistently demonstrated reductions in abdominal trunk fat and waist circumference, along with improvements in patient body image, without many of the adverse effects commonly associated with direct HGH administration.
